Greetings to you! I applaud you for taking an active interest in your recuperation and well-being. As an Injury Rehab Therapist, Functional Movement-Education Therapist, I am committed to your own healing process.
We both understand that you’re soon coming to see me for Movement Re-education and Therapy in order to alleviate your current discomfort and other issues. I know that the last thing you want to do when seeing a new practitioner or therapist for the first time is to "fill out paperwork".
That’s why I ask of my new clients, while being within the ease and comfort of your own home, to please complete a New Client Information Form (below).This is not at all mandatory. That is, if you would otherwise prefer to complete this form in my office during our session together, that is fine.
